Referral Information for CASES Programs During COVID-19

In this very challenging time in New York City, CASES’ programs continue to operate, with most services delivered remotely via phone and video and using online platforms. CASES’ services that are still accepting referrals and enrolling new clients include mental health clinic treatment, care management, family therapy, and youth employment and education services. NATHANIEL CLINIC

CASES’ Nathaniel Clinic continues to accept intakes, admit clients, and provide services, with a focus on individuals transitioning from inpatient and institutional settings, including those being released from hospitals, inpatient treatment, and jails and prisons

Most clinic services are now being delivered by phone or video. The clinic offers immediate availability for services such as psychiatric assessments along with help, as needed, for obtaining Medicaid. Clinic services currently available include: initial and psychiatric assessment, individual counseling, individual psychiatry, and health monitoring.

While the Nathaniel Clinic typically serves clients primarily residing in or near Harlem, the shift to telephonic counseling and therapy is bringing the clinic’s services to youth and adults citywide. Medication administration continues to be offered onsite at the clinic office in Central Harlem, along with the use of phones for clients who do not have one so they can engage in services with clinic staff.

HEALTH HOME PLUS CARE MANAGEMENT (HH+)

HH+ provides care management and support services to people in the community who have serious mental illness and chronic health conditions. HH+ continues to accept referrals, enroll clients, and provide care management.

Currently, most services are delivered by phone or video call, though the HH+ continues to conduct some essential services face to face while outfitted in full Personal Protective Equipment (PPE)

FAMILY THERAPY: ADOLESCENT PORTABLE THERAPY (APT)

APT is a flexible, in-home family therapy model that specifically serves justice-involved youth age 12-24 and their families. APT continues to accept referrals and enroll new clients, using phone, text, and video chat to deliver services. The program is well designed to help families facing relational and other challenges who are now spending extensive time at home together.

Typically lasting 4-6 months, APT is designed to identify, reinforce, and leverage a family’s existing strengths and assets to create buy-in and willingness to learn new relational skills and begin to address challenges.

YOUTH EDUCATION SERVICES:

JOBS FOR AMERICA’S GRADUATES (JAG NY)

JAG NY is an 18-month employment-readiness program serving young people age 17-24 who are out of school, unemployed, do not have a high school or equivalency diploma, and have been involved in the juvenile/criminal justice, foster care, and/or shelter systems. The program focuses on 20 core work-readiness competencies and provides direct support for securing and maintaining employment.

HIGH SCHOOL EQUIVALENCY (HSE) EXAM PREP

Most young people in CASES’ HSE services are age 17-24 and have current or previous involvement in the juvenile/criminal justice system. While HSE testing is paused until social distancing and social isolation orders are lifted, CASES continues to offer remote HSE prep classes to help prepare students to take the exam and earn their equivalency diploma, a key credential for seeking vocational training, securing employment, and/or enrolling in college.
