Primary Care at CASES Is Expanding!

Primary health care services at CASES’ Nathaniel Clinic in Central Harlem have expanded! Services are now available to the public—including all CASES’ participants along with their friends and family members every Wednesday from 9:00 AM–5:00 PM.

Dr. Weber, a female physician, covers the morning shift; and Dr. Weiss, a male physician, covers the afternoon to ensure patients have the opportunity to see a provider with whom they are comfortable.

The primary care services are provided by the Institute for Family Health (IFH), a community-based nonprofit that operates Federally Qualified Health Centers across New York City. They provide exceptionally high-quality care, and patients consistently report having positive experiences with IFH providers.

The Nathaniel Clinic is one of only two State-licensed outpatient clinics in New York City specializing in serving justice-involved individuals living with mental illness and the only one with this specialization licensed to serve youth (ages 13 and up). Physical health is critical to our participants’ well-being and is a common challenge for justice-involved persons. This expansion of our primary health care services provides easy onsite access in our offices.

Primary care services available include:

Check-Ups • Vaccinations • Physical Exams • STI Screening • Women’s Health Services • Gynecology Exams • PAP Tests • Family Planning • Men’s Health Services • Access to Specialty Medical Care • Injuries • Chronic Disease Management
