The Nathaniel Clinic Welcomes Tysen White, Care Management Team Leader

The Nathaniel Clinic welcomes Tysen White the Team Leader for the Health Homes Care Management Program. A new service offered by CASES, Care Management is the collaborative efforts between the member (or “client”) and service providers. Tysen will focus on our forensic population to facilitate the appropriate provision of health care services with the goal of achieving medical, social, and mental health stability.

Tysen enjoys motivating and empowering others. For the past 11 years, he has worked with children, teens, veterans, and older adults living with chronic medical illness and behavioral health conditions. In this new position at CASES, he plans to combine these professional experiences with what the skills and knowledge he gained while obtaining a Master of Social Work (MSW) from The Silberman School of Social Work at Hunter College and a license to practice social work (LMSW).

Tysen says his goal is to utilize care, compassion, and evidence-based practices to provide unrivaled services and to help guide CASES’ forensic population to successfully re-integrate themselves in the community in a positive way that reduces recidivism.

By working with the FACT and ACT teams, the Health Homes Care Management Program will serve as a step down in level of care and as the bridge between chronic levels of care and positive community integration.
